Information collected via the contact form
When you submit the contact form on this site, we collect the name, email address, optional phone number, optional reason category, and message you provide. We use that information only to reply to you. We don't sell or share it with third parties, and we don't add you to a marketing list without your consent.
Booking and Square
Appointment booking is handled by Square through an embedded widget on the Book page. When you book through that widget, Square collects the information needed to schedule the appointment (such as name, contact details, and selected service) and handles it under their own privacy policy. Their policy is available at squareup.com/us/en/legal/general/privacy.
Analytics
This site may, in the future, use a privacy-respecting analytics tool such as Google Analytics to measure aggregate traffic and page performance. If we add it, we will update this policy to say so. As of the last-updated date above, no third-party analytics are running on this site.
Cookies
This site does not set tracking cookies of its own. The embedded Square booking widget and any future analytics tool may set their own cookies under their own terms; review those tools' policies for specifics.
Patient privacy
In-clinic medical records are kept separately from this website and are not stored on this site or transmitted through it. The Body Lab handles patient health information in accordance with applicable state and federal privacy standards. This website itself does not collect, store, or transmit protected health information.
Changes to this policy
We may update this policy from time to time. The "last updated" date at the top of this page reflects the most recent revision. Material changes will be noted on this page; we don't email patients about routine policy updates.
